Membership FAQ

What forms of payment are accepted?

All forms of credit and debit cards including Visa, Mastercard, Discover and American Express.  

When can I start using my membership?

Your membership plan is effective immediately!

How do I schedule an appointment as a plan member?

Simply call our Modesto office and set up an appointment for yourself or your family members and mention that you are a Membership Plan member.

What happens if I miss an appointment?

It will be your responsibility to reschedule. Please see our office policy regarding no-shows and last-minute cancellations. Applicable fees will apply.

Can I add members to my account?

Absolutely! Your entire family can join our in-office discount plan.

Can I combine any other office discounts with the membership plan?

Membership plan discounts may not be combined with any other offers or discounts.

Can I utilize my membership at a Specialist office if I am referred to one by you?

No, our membership plan can only be utilized at the office of Downey Park Family Dentistry. But as a service to our patients, we do have Dental Specialists who work  from our office and your membership plan will apply.

Can I transfer my membership to another dentist?

Your membership is solely with Downey Park Family Dentistry and as such is non-transferrable.

How can I check on the status of my account?

Can I change or update my billing information?

Yes! Simply contact our office at (209) 529-0674 and we can assist you with that change.

How do I renew my membership when my year is up?

Our membership plan is an annual commitment and renews automatically on your anniversary date. You will receive an email to remind you that your plan is about to renew.

How do I cancel my membership?

Please contact our office at (209) 529-0674 or by email at office@downeyparkfamilydental.com 30 days prior to your auto renew.

If I cancel, can I get a refund on my membership?

No. Refunds will not be provided for dues paid under any circumstances, including failure to schedule, maintain appointments or utilize the plan.

How much do I pay to sign up for my plan membership?

The annual payment is due at time of sign up of the plan that applies to you.

What information is required for me to sign up for my plan membership?

An email address for receipts and renewal reminders, credit, debit card or bank account, full name, address, phone and date of birth.
